Resumo: Proliferative periostitis-osteomyelitis, also known as Garrè's osteomyelitis, is a form of inflammatory periosteal hyperplasia characterized by cortical plate reduplication, with an onion-skin appearance. This report describes a 10-year-old female patient who initially presented with right ankle pain and intermittent low-grade fever. A polymicrobial culture from a talus biopsy confirmed acute osteomyelitis, initially treated with a six-week antimicrobial regimen. Four months later, she returned with fever, chills, swelling in the mental region, local warmth, tenderness, and dysphagia. Clinical examination and panoramic radiography showed no dental or oral lesions. Cone-beam computed tomography revealed hypodense areas below the roots of teeth 46 to 36, with periosteal reaction, cortical thinning, rupture, and reduced medullary spaces. Findings suggested proliferative periostitis-osteomyelitis, confirmed by bone scintigraphy. The patient remained hospitalized under medical and dental supervision, receiving intravenous antimicrobials until infection control. Early diagnosis, imaging evaluation, and multidisciplinary management are essential for favorable long-term outcomes.
