Resumo: A 55-year-old female patient presented with a 7-month history of "swelling in the chin" and continuous pain. Extraoral examination revealed increased volume and redness in the mental region. Intraoral examination showed a poorly defined, reddish, expansive lesion in the anterior mandible, causing vestibular and lingual cortical bone expansion. Tooth displacement, mobility, and lingual ulceration were noted. A CT scan revealed an irregular hypodense area with cortical disruption and root resorption of the incisors, along with a hyperdense image resembling denticles. An impacted canine was observed below the lesion. Incisional biopsy revealed a sheet-like proliferation of small, round cells with hyperchromatic nuclei, pleomorphism, nucleoli, mitotic figures, and ghost cells. Immunohistochemistry showed positivity for AE1AE3, CK7, CK19, p40, p63, vimentin, and beta-catenin, with a high Ki-67 index. Negative markers included SMA, S100, CD3, CD20, and CD56. The diagnosis was odontogenic ghost cell carcinoma, and the patient was referred for treatment.
