Resumo: Osteoradionecrosis (ORN) is a radiation-induced ischemic bone necrosis associated with hypoxia, hypocellularity, and hypovascularity. Treatment includes radical and conservative approaches, such as photobiomodulation (PBMT) and antimicrobial photodynamic therapy (aPDT), which promote tissue repair and microbiological control. Extensive cases require invasive interventions, while conservative treatments are preferred whenever it is possible. A 68-year-old man with a history of oral cancer treated 6 years ago with radiotherapy and chemotherapy was referred for treatment bone sequestration in the jasw. He had no systemic alterations, was a smoker and former alcoholic. The examination revealed bone necrosis in the left lower border of the jaw. A panoramic radiograph was requested and laser therapy sessions with methylene blue were performed, with progressive improvement. Combined therapies have shown efficacy in controlling clinical lesions, improving symptoms, and regression of lesions. Although promising, more controlled studies are needed to confirm the scientific evidence.
