Resumo: Adenomatoid odontogenic tumor (AOT) is a rare benign epithelial odontogenic lesion, commonly found in the anterior maxilla of young females. Its association with an odontoma and occurrence in the posterior mandible are uncommon. This report describes a 50-year-old male patient with an incidental radiographic finding of a well-defined mixed radiolucent-radiopaque lesion in the posterior mandible. Histological examination after excisional biopsy showed odontogenic epithelium arranged in rosette-like structures, duct-like spaces, and calcified material confirming AOT. The odontoma component consisted of disorganized dentin, enamel, and cementum-like structures. The patient remains asymptomatic and under follow-up without signs of recurrence. This case highlights the atypical presentation of AOT in an older male, in an unusual posterior mandibular location and in association with an odontoma, reinforcing the importance of differential diagnosis for mixed radiographic lesions.
