Resumo: Chronic sclerosing sialadenitis (CSS) may be observed when evaluating intraoral nodular lesions or when evaluating minor salivary gland (mSG) biopsy in the diagnostic process of Sjogren syndrome (SS). When affecting the submandibular gland (Küttners tumor), it presents as a tumor mass, with some cases representing the spectrum of IgG4-related sclerosing disease. On microscopy, intense stromal sclerosis and parenchymal atrophy, permeated by florid lymphoplasmacytic infiltrate, preserving the lobular architecture, are typical. Subacute necrotizing sialadenitis (SANS) is an inflammatory disease of unknown etiology, affecting mSGs, and characterized histopathologically by atrophy and parenchymal necrosis permeted by mixed inflammatory infiltrate. A 39-year-old female patient presented with a submucosal nodule, slightly symptomatic on palpation, on the lower lip 5 weeks ago. After excisional biopsy, the diagnosis was SANS-like CSS. The current case expands the clinicopathological spectrum of chronic sialadenitis, emphasizing that a detailed histomorphological analysis is fundamental to achieve the correct diagnosis.
