Resumo: Myofibroma is a distinctive benign myofibrogenic neoplasm which can be present at birth or arise in adults, with predominance in males. This neoplasm often arises in the skin/subcutis of the extremities, head and neck and trunk. Microscopically, a biphasic growth pattern is observed, with primitive cellular zones surrounded by hyalinized nodules of myoid spindled cells. Surgery is the choice of treatment, with good outcomes. In the head and neck, in addition to the preferential cutaneous involvement of myofibroma, the jaws may be affected, preferably the mandible. Their clinical presentation exhibits a broad age range; however, there is predilection by the first decade of life. Here, we report an additional subcutaneous myofibroma affecting a 41-year-old man and located near the mandibular angle region. By immunohistochemistry, vimentin, α-SMA, and calponin were positive. p53 showed a wild-type pattern, and Ki-67 highlighted rare cells. After 1-year of follow-up, the patient is well, without recurrences.
