Resumo: Objective: To evaluate radiographic parameters associated with fractal dimension (FD), lacunarity (Lac), and grey-level pixel intensity in panoramic radiographs from patients with osteonecrosis. Study Design: This cross-sectional study included eighteen patients with active lesions in the maxilla or mandible, including twelve cases of osteoradionecrosis (ORN), and six cases of medication-related osteonecrosis of the jaw (MRONJ). A qualitative analysis was performed to select the optimal region of interest (ROI) (50×50 pixels) for quantitative evaluation, and the ROIs were defined in lesional (L) and non-lesional contralateral (NL) sites to be analyzed using ImageJ/Fiji software. Grey-level pixel intensities (mean-I_m, standard deviation-I_sd) were extracted from histograms, while FD and Lac were assessed using the FracLac plugin without binarization. Results: Eight patients were female, and ten were male, aged between 45 to 84 years old. The intraclass correlation coefficient for ROI selection was 0.99. No significant differences were found for FD (L=1.21±0.05; NL=1.24±0.05; p=0.09), Lac (L=0.1±0.03; NL=0.09±0.02; p=0.25), or I_m (L=120.2±26.2; NL=128.5±34.5; p=0.36). I_sd was higher (p=0.008) at L (10.61±3,46) compared to NL (8.33±3.15). Conclusion: Grey-level variation was higher in necrotic sites, showing an amplified pixel intensity variation with a wider range of values from black to white.
