Resumo: Actinic cheilitis (AC) is an oral potentially malignant disorder that primarily affects the lower lip. Given its malignant potential-especially its progression to squamous cell carcinoma-patients require close and continuous follow-up. Preventive care is essential and includes the regular use of lip balms with sun protection, as well as avoiding cosmetic procedures, which may mask or interfere with clinical evaluation. In this report, we present the case of a 52-year-old female patient previously diagnosed with AC with mild dysplasia. Despite the diagnosis, the patient underwent both lower lip micropigmentation and the application of lip filler. These aesthetic procedures altered the natural appearance of the lips, compromising the ability to monitor the lesion effectively during follow-up. Cases like this highlight the importance of awareness among professionals performing aesthetic procedures. It is critical that clinicians can recognize oral lesions, respect clinical contraindications, and prioritize the patient's health over aesthetic outcomes.
