Título: PERIPHERAL OSSIFYING FIBROMA: CASE REPORT.

Resumo: A 23-year-old female patient was referred for evaluation of an asymptomatic swelling in the lingual region of the lower anterior teeth. In her medical history, the patient reported having a diagnosis of lupus, high blood pressure, anemia, and "kidney problems." Due to the clinical characteristics and medical history, health care professionals referred her with a diagnostic hypothesis of brown tumor of hyperparathyroidism. No relevant changes were observed during the extraoral physical examination. Intraoral examination revealed a nodular lesion, with normal mucosal coloration and fibroelastic consistency, measuring approximately one centimeter in the largest diameter, in the lingual region of the lower anterior teeth, with an evolution time of approximately three years. The radiographic examination did not show any bone involvement. With the main hypothesis of gingival hyperplasia, an excisional biopsy of the lesion, which was inserted between teeth 41 and 42, was performed. Histologically, it presented fibrous connective tissue interspersed with bone trabeculae, establishing the diagnosis of peripheral ossifying fibroma. The patient remained under follow-up for six months without presenting recurrences. This report reinforces the importance of correlating information from the anamnesis, clinical examination and histopathological examination to differentiate gingival lesions, in this case particularly gingival hyperplasia, brown tumor of hyperparathyroidism and peripheral ossifying fibroma.
