Título: NECROTIZING ULCERATIVE GINGIVITIS SUPERIMPOSED ON MYELOID SARCOMA: A CASE REPORT

Resumo: A 41-year-old female patient diagnosed with acute myeloid leukemia developed a painful and purplish lesions with whitish areas on the gingival mucosa. A clinical diagnosis of granulocytic sarcoma was made. She was undergone to a chemo protocol with daunorrubicin and cytarabin. On day 5/7 of chemotherapy, she presented with necrotic interdental papillae extending to the attached palatal gingiva, diagnosed as necrotizing ulcerative gingivitis (NUG). Empiric antibiotics with metronidazole was prescribed, and she was already used meropenem for a bloodstream infection. After three days it was NUG progression. An incisional biopsy was performed for histopathological analysis, culture, and antibiogram, confirming Enterococcus faecalis sensitive to ampicillin and vancomycin but resistant to meropenem. Antibiotic therapy was switched to vancomycin, leading to clinical improvement after therapeutic adjustment, reached effective antibiotic treatment. NUG is becoming common in leukemia patients and need to further investigations in order to understand the etiopathogenesis and improve the treatament.
