Resumo: Oral squamous cell carcinoma (OSCC) is the most common malignancy of the oral cavity, with its progression influenced by the tumor microenvironment, particularly inflammatory responses. We report the case of a 64-year-old male with a history of heavy smoking for 50 years and chronic alcohol consumption, presenting with a rapidly growing 2.5 cm ulcerated lesion on the lateral tongue. Histopathological analysis revealed invasive nests of malignant squamous cells exhibiting marked nuclear pleomorphism, hyperchromatism, dyskeratosis, atypical mitoses, and an increased nucleus-to-cytoplasm ratio. A dense inflammatory infiltrate, including macrophages, multinucleated giant cells -an uncommon finding in OSCC- was observed at the tumor-stroma interface, suggesting an active immune response. The Ki-67 proliferation index of 20% indicated moderate proliferative activity. This case highlights the potential role of macrophage recruitment and multinucleated giant cell formation in tumor surveillance, suggesting an interaction between innate immunity and OSCC progression. The patient was referred for oncological management.
