Resumo: Acinic cell carcinoma (ACC) is a rare, low-grade malignant tumor of minor salivary glands, comprising less than 0.5% of cases. We report the case of a 50-year-old female with a 2 cm submucosal lip nodule, mobile on palpation, with a two-year history of progression. Her medical history includes esophageal disease and prior lung cancer treatment. Histopathological analysis revealed a glandular epithelial neoplasm composed of large cells with PAS/diastase-positive granules, and small eccentrically located nuclei. The tumor exhibited focal infiltration, microcystic spaces containing mucoid material and low mitotic activity. Immunohistochemical analysis showed positivity for AE1/AE3, CK7, and EMA, supporting the diagnosis of ACC of the minor salivary glands. Given the limitations of the incisional biopsy, further evaluation of the surgical specimen was recommended. The patient was referred for oncological management. In conclusion, ACC follows an indolent course, however recurrence rates can reach up to 35%, emphasizing the need for long-term monitoring.
