Resumo: An 80-year-old male patient with hypertension, prostate cancer with bone metastases, in use of Denosumab for three years. On physical examination, he presented bilateral lower molar implants, which had been made before cancer treatment. He had been using antibiotics monthly since he removed the implants for cleaning. Around the implant areas, he had gingiva with characteristics of acute inflammation, granulomatous appearance, intense, irradiated pain, without bone exposure. Computed tomography showed a diffuse, poorly delimited hypodense image around all the dental implants, with bilateral bone sequestration, compatible with medication-related osteonecrosis of the jaws (MRONJ). During the surgery to remove the implants, bone and soft tissue material was collected for anatomopathological analysis, confirming the diagnosis of MRONJ. The case reveals the importance of preserving the dental implants carried out previously and the patients occlusion after starting to use drugs that cause MRONJ.
