Resumo: A 54-year-old male presented with a seven-month history of multiple symptomatic ulcers affecting both the oral and genital mucosa. A prior histopathological evaluation of the genital lesion was inconclusive. Suspecting an autoimmune disease, the oral medicine team performed a cytopathological smear and an incisional biopsy. Cytological analysis revealed rounded keratinocytes with hypertrophic nuclei and occasional perinuclear halos, consistent with acantholytic (Tzanck) cells, within an inflammatory background. These findings provided an early indication of pemphigus vulgaris, later confirmed by histopathological examination, which demonstrated intraepithelial blister with preserved basal layer attachment (tombstone sign) and the presence of acantholytic keratinocytes. This case highlights the valuable role of cytopathology as a rapid and non-invasive diagnostic tool in pemphigus vulgaris, aiding early suspicion and guiding further confirmatory testing. The patient is currently under multidisciplinary management and receiving systemic prednisone therapy.
