Resumo: Proliferative Verrucous Leukoplakia (PVL) is a rare multifocal lesion of uncertain etiology, presenting higher prevalence in females (4:1) and individuals over sixty years of age. It mainly affects gingiva, ridge and palate, with a high risk of recurrence, malignancy and resistance to all types of treatment. This report describes an 83-year-old male patient, with no smoking and drinking habits, diagnosed with PVL. The patient reported oral burning and pain, with clinical signs of oral candidosis and white lesions on the floor of the mouth, tongue and gingiva. Candidiasis was treated with Daktarin and the burning sensation with laser therapy and artificial saliva. Incisional biopsies and panoramic radiographs were performed. Histopathological examination revealed Malpighian mucosa with acanthosis and moderate intraepithelial atypia. To confirm the diagnosis, immunohistochemistry was requested, which confirmed PVL. The patient is being followed up monthly to monitor the progression of the lesions and periodontal control.
