Resumo: Melanoma is a malignant neoplasm of melanocytic origin, typically affecting the skin but also occurring in mucosal areas, such as the hard palate and maxillary gingiva. This report presents the case of a 74-year-old woman with an extensive pigmented lesion on the floor of the mouth and mandibular gingiva. The patient reported that the lesion was removed a year ago, but it was not sent for histopathological analysis. An incisional biopsy revealed proliferating cells arranged in sheets separated by connective tissue. Atypical melanocytes were enlarged, exhibiting nuclear pleomorphism and hyperchromatism. Near the epithelium, large cells with dark brown pigment (melanin) were observed. Neoplastic cells were positive for S100 protein, and to a lesser extent for Melan A and HMB-45. The patient was referred to an oncologist. This case underscores the dentists role in early detection and diagnosis of rare, aggressive neoplasms in the oral cavity.
