Resumo: Primary intraosseous xanthoma affecting the jaws is an extremely rare entity classified within the broad group of histiocytic diseases. We report a case of a 47-year-old female who presented with complaints of pain and swelling in the right mandibular ramus, with a six-month history of progression. On intraoral examination, swelling was observed in the retromolar region and the right mandibular ramus. Computed tomography showed a multilocular hypodense lesion with a destructive appearance, extending throughout the mandibular ramus and causing cortical bulging and thinning. An incisional biopsy was performed, revealing numerous histiocytes within a myxoinflammatory background, with positivity for CD68 and negativity for S-100, confirming the diagnosis of xanthoma. Laboratory tests for cholesterol and triglycerides were within normal limits. Surgical excision of the lesion was performed via a submandibular approach, followed by curettage and peripheral osteotomy. The histopathological examination of the final specimen confirmed the diagnosis.
