Resumo: The oral microcystic lymphatic malformation is a benign vascular lesion characterized by dilated lymphatic channels with a favorable prognosis. A 37-year-old woman presented with the main complaint of a tongue lesion with six-month evolution. Physical examination revealed a firm, purplish nodule, asymptomatic showing an ulcerated surface partially covered by a hemorrhagic crust, located in the right posterior dorsum of the tongue, measuring 2 cm in diameter. The differential diagnoses included pyogenic granuloma and vascular malformation. An excisional biopsy was performed, and histopathological analysis showed oral mucosa consisting of a parakeratinized hyperplastic stratified squamous epithelium adjacent to an ulcerated area covered by a serofibrinous pseudomembrane and polymorphonuclear cells. Additionally, diverse lymphatic vessels of varying sizes containing proteinaceous material, hemorrhagic areas and mononuclear inflammatory infiltrate were also observed. The final diagnosis was superficial microcystic lymphatic malformation. The patient was informed about her condition and the six-month follow-up showed no signs of recurrence.
