Resumo: Peripheral ameloblastoma is a rare odontogenic tumor mostly found in the mandibular premolar gingiva. A 55-year-old woman was referred with the main complaint of a gingival nodule with an evolution time of seven years. Physical examination revealed a well-defined, normochromic nodule with a smooth surface, sessile base, and asymptomatic in the region of teeth 43 and 42, measuring 1.5 cm. No local radiographic changes were observed. The differential diagnoses included cemento-ossifying fibroma and peripheral giant cell lesion. An excisional biopsy was performed, and histopathological analysis revealed cords and islands of odontogenic epithelium with peripheral palisaded, hyperchromatic cells showing cytoplasmic vacuolization and reverse polarization, diffusely distributed in a fibrous connective tissue. Additionally, areas of squamous metaplasia and continuity between the tumor epithelium and the oral mucosa were observed. The final diagnosis was peripheral ameloblastoma. The patient was informed about her condition and the one-year follow-up showed no signs of recurrence.
