Título: Osteoblastoma of the mandible in a 3-year-old child: A case report

Resumo: A 3-year-old male presented with a 4-month history of progressive facial swelling. Extraoral examination revealed asymmetry and increased facial volume. Intraorally, firm swelling in the vestibular sulcus extending from teeth 73 to 75 was observed. CT imaging showed a well-defined, expansile, hypodense lesion with internal calcifications and cortical bone expansion, measuring 30×31 mm. Incisional biopsy revealed a mesenchymal proliferation but was deemed non-representative. Due to the puzzling diagnostic features, a second biopsy was performed two months later-after the lesion had enlarged to 41×38×43 mm. Histopathological analysis showed atypical cells with large nuclei, abundant cytoplasm, diffuse calcifications, neoplastic osteoid, and peripheral giant cells, leading to a final diagnosis of osteoblastoma. Although rare in facial bones, especially in children under 10, osteoblastoma may present local aggressiveness and atypical histopathology despite its benign nature. The patient underwent conservative surgical excision under general anesthesia, with no signs of recurrence at a three-month follow-up.
