Resumo: Psoriatic arthritis (PsA) is a chronic inflammatory disease similar to rheumatoid arthritis, ankylosing spondylitis, and reactive arthritis. Diagnosis requires detailed evaluation, as symptoms vary over time. The most common oral manifestations include periodontitis, fissured tongue, temporomandibular changes, and, rarely, involvement of the labial mucosa. A 54-year-old woman sought care due to "recurrent scaling of the lips." She reported no systemic diseases. Intraoral examination revealed scaling and crusting of the lower lip. Incisional biopsies indicated epithelial hyperplasia, acanthosis, and superficial inflammatory infiltrate, diagnosing exfoliative cheilitis and ruling out Sjogrens syndrome. Treated with tracholimus 0.1%, the condition improved, but the burning and redness of the eyes returned after 2 months, and the medication was discontinued. She was referred to a rheumatologist, who ordered tests and diagnosed psoriatic arthritis (PA) associated with exfoliative cheilitis as an oral manifestation. Methotrexate was started and the patient remains stable, highlighting the importance of a multidisciplinary approach.
