Resumo: A 36-year-old female, a teacher with a history of bariatric surgery and hormonal intrauterine device use, presented for the extraction of teeth 28 and 38 as part of orthodontic treatment. Denies previous illness or use of medication. The aseptic surgery was performed without complications using standard techniques I and II. Postoperative care included Metamizole 1g and Nimesulide 100mg every 12 hours. Approximately 30 hours after the procedure, the patient returned with symptoms including fever, unilateral facial pain, tenderness upon palpation of lymph nodes, dysphonia, trismus, and submandibular swelling with erythema. Immediate oral administration of Dexamethasone 8mg was initiated and maintained for five days. Additionally, Amoxicillin + Potassium Clavulanate (1750mg + 250mg) was prescribed every 12 hours for three days, followed by a reduced dose of 875mg + 125mg for an additional two days. Metamizole was continued. The patient was monitored every 24 hours and showed progressive improvement.
