Resumo: Odontogenic keratocyst (OKC) is a common developmental odontogenic cyst affecting the maxillofacial region. Usually, multiple OKCs occur as a component of nevoid basal cell carcinoma syndrome (NBCCS) with concomitant cutaneous, skeletal, ophthalmic, and neurologic abnormalities, but 5% of patients with OKC have multiple cysts without concomitant syndromic presentation. We present the case of a 14-year-old girl with a previous history of at least 4 OKCs removed from her jaw around 5 years ago. At the time, the CT showed a large expansive osteolytic process in the left posterior maxilla, and a smaller one in the anterior region. There was also a suggestive image in the left posterior region of the jaw. Surgery consisted of excision and curettage of the cystic lesions. Periodic radiographic controls and referral to other medical specialties to investigate NBCCS were followed and the patient is undergoing postoperative follow-up without signs of recurrence.
