Resumo: The buccal bifurcation cyst is a rare inflammatory odontogenic cyst that typically develops on the buccal area of the mandibular permanent first molar. This report presents the case of a 7-year-old male patient, who had painful clinical swelling in the buccal mucosa of the lower left permanent first molar. Periodontal probing demonstrated pocket formation. Periapical radiograph showed a well-circumscribed unilocular radiolucency involving the buccal bifurcation, and cone-beam computed tomography demonstrated the exact buccal location of the lesion. With the hypothesis of an inflammatory odontogenic cyst, excisional biopsy was performed. Histopathological examination revealed a cyst capsule lined by stratified squamous epithelium with exocytosis and spongiosis along with an underlying connective tissue presenting mononuclear inflammatory cell infiltrate. Clinical, radiograph and histopathologic aspects were compatible with buccal bifurcation cyst. Thirty-days after biopsy, patient had no pain or any sign of lesion recurrence. This report highlights the diagnostic challenges associated with buccal bifurcation cyst.
