Resumo: Traumatic ulcerative granuloma with stromal eosinophilia (TUGSE) is an uncommon self-limiting ulcerative lesion that may clinically mimic oral squamous cell carcinoma (OSCC). We herein report four illustrative new cases of TUGSE diagnosed in a Brazilian Oral Pathology laboratory between 2018 and 2023. There were three women and one man with ages ranging from 24 to 82 years old. Three cases presented as an ulcerated reddish to whitish nodule in the lateral border (1 case), ventral (1 case), and dorsal (1 case) surfaces of the tongue and posterior buccal mucosa. OSCC was the main clinical diagnosis of all cases, which were submitted to incisional biopsy. The gross appearance of all cases revealed an exophytic whitish fragment of fibrous consistency. Microscopically, an ulcerated nodule with the predominance of chronic mononuclear inflammatory cells and a variable number of eosinophils was observed. Partial resolution of one lesion was observed a few days after incisional biopsy, and there was no recurrence. Oral TUGSE may clinically appear as long-standing exophytic ulcerative lesions, which should be submitted to microscopic evaluation to rule out OSCC.
