Resumo: Squamous cell carcinoma is the most prevalent mouth cancer and, when it arise in the tongue, asymptomatic or clinically aggressive cases depending on the tumor stage being initial or advanced, respectively. The present case affected a 40-years-old, non-alcoholic and non-smoker female presenting an ulcerated lesion with history of physical trauma due to the biting during sleep. On clinical examination, a painful excavated ulcer was observed, with erythroplastic borders on the right edge of the tongue, measuring approximately 1cm. With the diagnosis of a traumatic ulcer, corticosteroid therapy and topical antifungals were prescribed but without improvement after 15 days. The patient returned without healing with an increase in size and pain of the lesion. A new biopsy was performed and the diagnosis was squamous cell carcinoma. The patient was referred for treatment with partial glossectomy, neck dissection, radiotherapy and chemotherapy. Currently, the patient has been follow-up and disease-free for 2 years.
