Título: EVALUATION OF HISTOPATHOLOGIC GRADING SYSTEMS IN EPITHELIAL DYSPLASIAS AND SQUAMOUS CELL CARCINOMAS OF THE LOWER LIP

Resumo: Objective: To analyze cases of epithelial dysplasias (EDs) present in actinic cheilitis (AC) and lower lip squamous cell carcinomas (LLSCC) based on four morphological grading systems, as well as to investigate the prognostic impact of two of those systems in LLSCC. Study Design: Thirty-nine EDs and 45 LLSCCs were included in this study. AC cases were analyzed based on morphological criteria determined by the 4th classification of World Health Organization (WHO) and the binary system proposed by Kujan et al. (2006). LLSCC cases were graded according to WHO (2017) criteria and the BD model proposed by Almangush et al (2015). Results: Analysis of ACs based on the binary system showed a slight predominance of high-risk grading (51.3%). Based on WHO criteria, most cases were exhibited moderate dysplasia (29.2%), and of these, most were graded as high risk (64.2%) in the binary system. For LLSCC cases, a predominance of moderately differentiated carcinomas were observed (53.3%) according to WHO criteria. Regarding the BD model, most cases exhibited intermediate risk (44.4%) and death was a significantly frequent outcome in those with histopathological high-risk score (p = 0.048). Conclusion: The BD model was the only histopathological gradation system associated to outcomes in LLSCC patients.
