Resumo: The central odontogenic fibroma is a rare benign neoplasm. An 18-year-old female patient was referred to Oral and Maxillofacial Surgery service due to a radiographic finding. In the intraoral examination, the patient presented volume increase in the posterior region of the left maxilla and tooth displacement. Computed tomography showed a hypodense, expansive image with destruction of bone cortices and displacement of adjacent teeth. An incisional biopsy was performed and the histological report and immunohistochemical profile were compatible with odontogenic fibroma. The treatment proposed was enucleation/exeresis of the lesion and exodontia of the teeth involved. After 8 months of follow-up, bone reconstruction with mandibular oblique line graft was proposed. The patient is under follow-up with no signs of recurrence. Although the odontogenic fibroma does not have a capsule, it detaches easily and conservative surgical treatment with vigorous curettage has shown good results, with low recurrence rates.
