Resumo: Odontogenic keratocyst is an epithelial-origin cyst characterized by aggressiveness and a high rate of recurrence. It predominantly affects the posterior region of the mandible, often associated with an impacted third molar. In this case, a 24-year-old male patient presented with a radiographic finding and complained of sensitivity to palpation in the posterior region of the left mandible. No alterations were observed during intra and extraoral clinical examinations, except for displacement of impacted teeth 37 and 38. Radiographic imaging revealed a radiolucent image with defined limits and a radiopaque halo, extending in the anteroposterior mandibular direction. An incisional biopsy confirmed the diagnosis of odontogenic keratocyst. A 3D reconstruction was utilized for surgery planning, and enucleation and curettage with chemical cauterization using 5-fluorouracil cream were performed. During the 12-month postoperative period, the patient reported no complaints or signs of recurrence. However, prolonged follow-up is essential for the early detection of recurrences.
