Resumo: Secretory carcinoma, also known as mammary analogue secretory carcinoma, is a salivary gland neoplasm that morphologically and immunohistochemically resembles secretory carcinoma of the breast and shares the ETV6 gene rearrangement. This lesion was first described in 2010 and affects patients aged 10 to 86 years, with a mean age of 46.5 years. A 21-year-old white male presented with an asymptomatic slow-growing mass with mucosal ulceration on the soft palate. The clinical diagnosis was adenoma pleomorphic, and an incisional biopsy was performed. Histopathological examination showed a proliferation of cells with round to oval, sometimes vesicular nuclei, centrally located nucleoli and pale eosinophilic cytoplasm. The tumor predominantly exhibited a solid growth, in addition to areas of tubular and microcystic structures with PAS-positive bubbly secretory material. The immunohistochemical study demonstrated positivity for S100 protein, mammaglobin and vimentin. The final diagnosis was secretory carcinoma, and the patient was referred to an oncologic service.
