Resumo: Langerhans cell histiocytosis (LCH) is a benign but locally destructive rare neoplasm that mainly affects the pediatric population, ranging from a single osseous lesion to multisystem organ dysfunction, with head and neck region being involved in more than half of the cases. A 4-year-old male patient was referred for diagnosis of lesions located in both the posterior maxilla and mandible. On intraoral examination, the involved teeth presented severe mobility with erythematous adjacent gingiva, significant accumulation of biofilm and calculus, and fetid odor. The patient also presented facial and lower right eyelid swelling and episodes of draining purulent bloody discharge. Multiple osteolytic lesions were observed in the imaging exams. An incisional biopsy was performed and the histopathological and immunohistochemical features confirmed the diagnosis of LCH. Although this is a rare neoplasm, it should be considered in the differential diagnosis of periodontitis-type lesions and floating teeth in pediatric patients.
