Resumo: Odontogenic keratocysts (OK) are benign odontogenic cysts, with aggressive behavior and are highly recurrent. A 39-year-old female patient was referred to the oral surgery service after a radiographic finding. During the physical examination, she reported a history of breast cancer six years ago, without bone metastases or use of antiresorptive medications. No intraoral or extraoral changes were observed in the clinical examination. Imaging exams showed a well-defined unilocular radiolucent area located in the region distal to tooth 37. An incisional biopsy was performed, and the diagnosis was OK. The treatment performed was enucleation and curettage of the lesion associated with chemical cauterization using 5-fluorouracil, which was kept in the cavity with a gauze and removed after 24 hours. The patient has been under radiographic follow-up for 12 months without recurrence. Complementary treatment after lesion removal reduces the recurrence of OK, and 5-fluorouracil is an effective option.
