Resumo: Osteosarcoma of the jaws is a relatively rare neoplasm, accounting for 2-10% of all cases of osteosarcoma, characterized by aggressive local growth and early metastatic potential. A 43-year-old man presented with a 5-month history of an osteoma in maxilla. Intraoral examination revealed a hard tumor, with reddish and ulcerated surface, located in the anterior region of maxilla. Computed tomography scan disclosed an expansive, heterogenous lesion, with undefined borders, extending from tooth #13 to #23. Considering the initial diagnosis of osteoma, an excisional biopsy was performed. Histopathologic examination revealed a mesenchymal malignant neoplasm characterized by proliferation of spindle cells with high index of nuclear hyperchromatism and atypical mitoses. Osteoblastic and chondroblastic patterns were evidenced. Final diagnosis was osteosarcoma. The patient underwent radical surgery and follow-up of 31-month showed recurrence. This case emphasizes the importance of a correct diagnosis in early stage to adequate treatment, promoting better quality of life.
