Resumo: Odontogenic keratocyst is a benign, aggressive cyst that often recurs and mainly affects the mandible. A 28-year-old female patient was referred to the oral surgery service due to a radiographic finding. Clinical examination showed a slight increase in volume in the left retromolar trigone region, detected only by palpation. Imaging exams, a well-defined, unilocular radiolucent image was observed, extending from the distal region of tooth 38 in the ascending mandibular branch to the coronoid process, with areas of cortical bone erosion and involvement of the mandibular canal. The treatment performed consisted of extraction of tooth 38, followed by enucleation and curettage of the lesion associated with chemical cauterization using 5-fluorouracil cream, which was kept in a surgical site on a sterile gauze and removed after 24 hours. After nine months of follow up, there are no signs of recurrence. Complementary treatment of the surgical site is essential to reduce recurrence.
