Resumo: Leishmaniasis is a zoonosis of rare primary involvement of the oral mucosa. Male patient, 49 years old, referred to the stomatology outpatient clinic with an ulcerative lesion in the labial commissure. The patient denied bodily injury, comorbidity, without medical follow-up. On intraoral physical examination, a moriform ulcerated lesion with irregular borders was observed in the skin and mucosa of the upper lip, right jugal commissural region, painful symptoms and 3 months of evolution. An incisional biopsy was performed and the material sent for anatomopathological examination, in which lymphoplasmacytic inflammatory infiltrate and multinucleated giant cells were observed, with granulomatous formations, as well as cylindrical structures in the macrophage and extracellular cytoplasm positive for periodic acid-Schiff (PAS) and Grocott, concluding the diagnosis of leshimaniasis. The patient was referred for treatment.
