Resumo: Botryoid odontogenic cyst (BOC) is a rare lesion, with only one hundred cases reported after 43 years of its initial description. BOC is a developmental odontogenic cyst, considered a variant of the lateral periodontal cyst. We report a case of a female patient, 73 years-old, who presented with an asymptomatic intraosseous lesion discovered in a routine radiographic examination. It was a radiolucent, multilocular, well-circumscribed lesion located at the distal region of lower right canine tooth. With the hypothesis of odontogenic keratocyst, enucleation was chosen. Histopathological analysis resulted in the diagnosis of BOC, revealing distinct cystic structures, both lined by thin squamous epithelium with some areas of nodular thickening and clear cells, supported by a capsule of dense connective tissue. Patient has been followed up for seven months, with no recurrence.
