Resumo: Odontogenic myxoma (OM) is a rare benign neoplasm that accounts for less than 5% of odontogenic tumors. Hereby we report the case of a 57-year-old female showing an asymptomatic tumor in the right maxilla. Imaginological exams revealed a well-delimited multilocular lesion, with thin bone septa at right angles, occupying the right maxillary sinus and extending from tooth 16 to 22. An incisional biopsy revealed hypercellular myxoid connective tissue. Immunohistochemical analysis showed diffuse positivity for vimentin, but focal immunoreactivity for S-100 protein, α-SMA, and CD34. The histopathological diagnosis was OM. A total right and partial left hemimaxillectomy were performed. The patient has been rehabilitated and under follow-up for seven years without recurrence. Despite its benign nature, OM can present an aggressive clinical evolution due to its progressive infiltrative growth. In this work, a discussion on the different aspects of its biological behavior and diagnostic criteria is provided.
