Resumo: Multiple myeloma is multifocal proliferation of plasma cells in the bone marrow, in which neoplastic cells replace normal marrow cells, produce excess immunoglobulins and infiltrate the bone. Plasmacytoma can present in the oral cavity as solitary bone or rarely, less than 1%, as extramedullary when it affects soft tissues. Female patient, 64 years old, with a diagnosis of osteolytic lesion in the knee region of an unidentified primary tumor, complaining of tingling and numbness in the mouth, in the left mandibular region radiating to the lip. On clinical examination, the presence of a 2-cm expansive nodular lesion in the periapical region of the lower left premolar and molar teeth was noted. In radiography there are multiple radiolucent images in different areas of the mandible and maxilla. The biopsy of the oral lesion was performed. Microscopic analysis and immunohistochemical study with CD138 and Kappa concluded the diagnosis of plasmacytoma; multiple myeloma.
